Text messaging is not intended for urgent, emergent, or time-sensitive medical issues.

User Eligibility and Responsibilities

By using our messaging services, you confirm that:

  • You are at least 18 years of age or the legal guardian of the individual receiving care

  • You are authorized to use the mobile number provided

  • You agree not to use text messaging for emergencies or urgent medical needs

You are responsible for notifying Provita Wellness, LLC of any changes to your mobile number.

Consent and Opt-In

You consent to receive text messages from Provita Wellness, LLC by providing your mobile number and affirmatively opting in. Consent may be obtained verbally, in writing, electronically, or by initiating a text conversation with the practice.

Message frequency may vary. Message and data rates may apply.

Opt-Out and Support

You may opt out of receiving text messages at any time by replying STOP to any message.
